Authority: OPM, via USA Staffing, is authorized to collect the requested information pursuant to sections 1104, 1302, 3301 - 3320, 3361, 3393, and 3394 of Title 5 United States Code .

Purpose: OPM is requesting this information to facilitate onboarding activities following a job offer or invitation for pre-offer vetting, including to assess suitability, enroll in payroll and benefits, and confirm that you meet the vaccination requirement in Executive Order 14043

Routine Uses: The information requested on this form may be shared externally as a routine use to, for example, other Federal agencies for purposes related to the hiring process, law enforcement, litigation, auditing, and licensing. A complete list of the routine uses can be found in the systems of records notice associated with this collection, OPM/GOVT 5 Recruiting, Examining, and Placement Records, 79 FR 16834, 80 FR 74815.

Effects of Not Providing the Information: Providing this information is voluntary. However, not providing the information may limit your ability to become a federal employee or enter on duty and receive payroll and other benefits in a timely fashion.

Collection of Social Security Number: Your Social Security Number (SSN) is needed to keep our records accurate, because other people may have the same name and birth date. Public Law 104-134 (April 26, 1996) asks Federal agencies to use this number to help identify individuals in agency records. Giving us your SSN is voluntary. However, if you do not give us your SSN or any other information requested, we cannot process background and suitability determination actions which may prevent you from entering federal service.

Public Burden Statement: OPM estimates that it will take on average 25 minutes per person to complete, including the time for reviewing instructions, obtaining the necessary information, and reviewing the completed form.
